苯那普利对2型糖尿病性肾病患者血浆一氧化氮和内皮素影响的研究
The lnfluence of Benazepril on the Nitric Oxide and Endothelin in Type 2 Diabetic Nephropathy
【摘要】 目的 :研究一氧化氮 (NO)和内皮素 - 1(ET - 1)在 2型糖尿病性肾病 (DN)患者中的变化及苯那普利对其影响。方法 :选取 2型糖尿病 (DM)患者 119例 (男 5 6例 ,女 6 3例 )和正常对照组 30例 (男 16例 ,女 14例 ) ,抽取静脉血测NO和ET - 1水平。结果 :2型DM患者血浆NO明显低于正常人 (P <0 .0 5 ) ,血浆ET - 1和血管紧张素Ⅱ(AngⅡ )明显高于正常人 (P <0 .0 1) ;2型DN组血浆NO明显低于 2型DM组 (P <0 .0 1) ,血浆ET - 1和AngⅡ明显高于 2型DM组 (P <0 .0 1)。 2型DN组NO与ET - 1、AngⅡ、尿白蛋白排泄率 (UAER)呈负相关 ,ET - 1与AngⅡ、UAER呈正相关。苯那普利治疗后血浆NO明显升高 (P <0 .0 5 ) ,而ET - 1和AngⅡ则明显下降 (P <0 .0 5或P <0 .0 1)。结论 :NO与ET - 1失衡在DN的发生发展中起重要作用 ;苯那普利对肾脏有保护作用
【Abstract】 Objective:To investigate the changes of nitric oxide(NO) and endothelin-1(ET-1) levels in patients with type 2 diabetic nephropathy(DN) and the regulation of benazepril on NO and ET-1.Methods:Plasma NO and ET-1 level of 119 type 2 diabetic patients(56 males and 63 females) and 30 healthy controls (16 males and 14 females) were measured.Results:NO level in type 2 DM were significantly decreased than that in control( P < 0.05 ).ET-1 and angiotensin Ⅱ (Ang Ⅱ)levels in type 2 DM were significantly higher than that in control( P <0.01).NO level of type 2 DN were significantly lower than that of type 2 DM( P <0.01).ET-1 and Ang Ⅱ levels of type 2 DN were significantly higher than that of type 2 DM( P <0.01).There was negative correlation between plasma levels of NO and concentrations of ET-1,Ang Ⅱ or urinary albumin excretion rate(UAER).There was positive correlation between plasma levels of ET-1 and concentrations of Ang Ⅱ or UAER.NO level increased.ET-1 and Ang Ⅱ levels decreased after benazepril treatment( P <0.05). Conclusion: NO and ET-1.May play an important role on development and progression of type 2 DN.Benazepril may have some protective effect on the kidney.
