【摘要】 目的评价聚合酶链反应-序列特异性引物(PCR-SSP)法检测HLA-B27基因对强直性脊柱炎(AS)的临床诊断价值。方法对87例临床疑似AS患者按MNY标准诊断分为病例组(AS组)和对照组(非AS组),并以PCR-SSP法检测患者血样HLA-B27基因,盲法比较二者结果。结果病例组和对照组HLA-B27阳性率分别为92.86%和11.11%,差异有统计学意义(P<0.05)。HLA-B27基因检测对AS诊断的敏感度(Sen)为92.8%、特异度(Spe)为88.9%、准确度(Acc)为90.8%、阳性预测值(+PV)为88.6%、阴性预测值(-PV)为93.0%、阳性似然比(+LR)为8.4、阴性似然比(-LR)为0.08,比值比(OR)为104.0。结论该法检测HLA-B27基因对AS诊断有较高敏感度和特异度,是一种准确、有效的实验诊断方法。更多还原

【Abstract】 Objective To appraise the diagnostic value of human leukocyte anti-gen B 27 gene(HLA-B 27 gene)detection for ankylosing spondylitis(AS)by PCR-SSP technique.Methods Among87patients seeming AS,42cases were merged into AS group and45cases(non-AS)into control group according to MNY diagnostic standard,and thair HLA-B 27 gene in blood were determined by PCR-SSP method.The results both of the groups were compared by Blind-method.Results The HLA-B27positive rates of the AS and control group,were92.86%and11.11%respectively.It was showed significantly different (P<0.05).The determination of HLA-B 27 gene for the AS showed:the sensitivity was92.8%,with the specificity88.9%,the accuracy90.8%,the positive predictive value88.6%,the negative predictive value93.0%,the positive likelihood ratio8.4,the negative likelihood ratio0.08and the odds ratio104.0respectively.Conclu-sions It is suggested that the determination of HLA-B 27 gene for the diagnosis of AS is an accurate and effective experimental diagnostic method as its highly sensitivity and specificity.更多还原
