脑血吸虫肉芽肿的影像学诊断

【摘要】 目的 探讨脑血吸虫肉芽肿的影像学特征。方法 回顾性分析了 6 0例病理或临床证实的颅脑血吸虫肉芽肿的影像学改变 ,6 0例进行了 CT检查 ,12例进行了 MRI检查。结果 病例共有病灶 75个 ,大多数位于皮层或皮层下 ,此外还可见于小脑半球、胼胝体。根据病灶大小 ,脑血吸虫肉芽肿表现为结节状或肿块状病变。其中 ,结节性病灶 45个 ,呈等密度或等信号结节伴周围不同程度指状水肿 ,42个病灶均匀强化。 3个病灶环形强化 ;肿块状病灶 30个 ,CT平扫呈等或稍高密度 ,MRI呈稍长 T 1、稍长 T 2或等 T 1、等 T 2信号 ,其中 2 9个病灶呈成簇堆积的结节状强化 ,病灶周围水肿和占位效应明显。结论 脑血吸虫肉芽肿有其自身的影像特征 ,可单发或多发 ,可列入颅脑单发或多发病变的鉴别诊断之中。

【Abstract】 Objective To characterize the imaging features of the cerebral schistosomiasis. Methods 60 cases with pathologically or clinically-proved cerebral schistosomiasis granuloma were retrospectively analyzed. All 60 patients were evaluated with CT and 12 were studied by MR imaging. Results The total number of the lesion found in CT and MRI was 75, and most of lesions located in the cortical or subcortical areas. According to its size, cerebral schistosomiasis granuloma appeared as nodular or mass-like lesions. 45 nodular lesions appeared as a small isodense or iso-intense nodule surrounded by marked finger-shaped perifocal edema. There were 42 demonstrated homogenous enhancements except three ring-like enhancements. 30 lesions were mass-like. On plain scan, it appeared as isodense or slightly hyperdense foci. On MRI, it appeared as a slightly long T 1 and long T 2 or iso-T 1, iso-T 2 signal, among which, 29 resulted from the confluence of multiple patchy clustered enhancing-nodules. Conclusions The cerebral schistosomiasis granuloma has its own imaging features. It might be involved in the differential diagnosis for multiple or solitary nodular and mass-like lesions in the brain, especially in epidemic areas.
