前列腺素E1治疗犬油酸型急性肺损伤的实验观察

【摘要】 目的 观察前列腺素E1 联合呼气末正压 (PEEP)对犬油酸型急性肺损伤 (ALI)的治疗作用。方法  1 5只成年健康家犬随机分成 3组 ,静脉注射油酸复制性肺损伤模型后分别给予PGE1 雾化吸入或静脉滴注或雾化吸入生理盐水 ,同时联合PEEP治疗。结果 PGE1 治疗组犬动脉血氧分压 (PaO2 )、氧合指数 (PaO2 /FiO2 )均明显改善 ,肺循环阻力(PVR)和平均肺动脉压 (MPAP)均显著降低 ;且雾化吸入组动物体循环平稳 ,静脉治疗组体循环指标变化显著。结论 雾化吸入PGE1 可选择性扩张肺血管 ,降低油酸型ALI犬MPAP和PVR ,迅速提高PaO2 /FiO2 ,且不影响体循环

【Abstract】 Objective To observe the therapeutic effect of prostalandin E 1 (PGE 1) and post end-expiratory pressure (PEEP) on oleic-acid induced acute lung injury (ALI) in dogs.Methods 15 healthy adult mongrel dogs were randomized into 3 groups. After ALI models was successfully established by injecting oleic-acid through central vein hole of Swan-Ganz catheter, PGE 1 were nebulized or infused with PEEP.Results Pressure of artrial oxygen partial(PaO 2)and oxygenation index(PaO 2/FiO 2)were greatly improved, mean pulmonary arterial pressure (MPAP) and pulmonary vascular resistance(PVR) were greatly decreased during therapy. Systemic hemodynamics were stable during PGE 1 nebutization, but systemic hemodynamics greatly changed during PGE 1 infusion.Conclusion PGE 1 nebulization can selectively dilate pulmonary vascular, reduce MPAP and PVR of oleid-acid induced ALI dog models, and rapidly improve PaO 2 and PaO 2/FiO 2 without affecting systemic hemodyanmic.
