【摘要】 目的 :探讨贲门癌穿孔患者的外科治疗方法。方法 :对住院治疗的 2 1例贲门癌穿孔患者的临床资料进行回顾性总结分析。结果 :住院死亡 3例 ,术前明确诊断 9例 ,一期行近端胃大部切除术 11例 ,全胃切除术 4例 ,瘘口大网膜堵塞缝合 6例 ,发生吻合口瘘 2例 ,行近端胃大部切除者 >3a生存者 6例 ,>5a生存者 3例。行单纯修补的均 <1a死亡。结论 :对贲门癌穿孔患者 ,只要生命体征平稳 ,全身状态许可 ,行一期近端胃大部切除术是可行的 ,也是对此类患者较好的一种治疗方法。更多还原

【Abstract】 Objective To explore surgical treatment of the patients with perforating cardiac carcinoma.Methods The clinical materials of 21 cases of perforating cardiac carcinoma were retrospectively analysed.Results Nine cases were definitely diagnosed before operation.One stage proximal gastrectomy was performed in 11 cases,total gastrectomy in 4 cases and leakage stitched and blocked with omentum in 6 cases.Anastomosis leakage occurred in 2 cases.Three patients died during hospitalization.Six patients who underwent proximal gastrectomy survived for more than three years,among whom three patients survived for more than five years.The others died within one year after operation.Conclusion It is feasible for the patients with perforating cardiac carcinoma to be performed one stage upper gastrectomy or total gastrectomy if life signs are steady and general status is permitted.One stage resection is a good treatment for the patients with perforating cardiac carcinoma.
