双冲击疗法治疗活动性狼疮性肾炎的临床疗效

【摘要】 目的评估甲基强的松龙 (MP)及环磷酰胺 (CTX)双冲击治疗活动性狼疮性肾炎 (LN)的疗效和安全性。方法将 6 9例活动性狼疮性肾炎随机分成两组进行治疗 ,分别为双冲击治疗组 (n =39)和标准激素加CTX治疗组 (n =30 ) ,并观察比较病人血清抗双链 DNA、补体C3、血肌苷、和 2 4h尿蛋白定量等实验室指标变化及疗效和不良反应。结果双冲击组总有效率 (81 % )优于标准激素加CTX治疗组 (5 1 % ) ,两组比较差异有显著性 (P <0 .0 5 ) ,前者的实验室指标较后者明显改善 ,两组间的不良反应发生率无明显差异。结论双冲击疗法是治疗活动性狼疮性肾炎的一种安全、有效的方法

【Abstract】 ObjectivesTo evaluate the therapeutic efficacy and safety of double pulse therapy (DPT) with methylprednisolone (MP) plus cyclophosphamide (CTX) in the treatment of active lupus nephritis (ALN).MethodsSixty nine patients of ALN were randomly devided into two groups:①Group A , thirty nine patients were treated by DPT ;②Group B, 30 patients were treated with standard corticosterone combined CTX therapy. Changes of laboratory indices, including serum levels of anti double strain DNA antiboby (AdsDNA),. C3, creatinine and protein quantitation in 24 hours urine as well as the therapeutic efficacy and side reactions in both groups were observed and compared .ResultsThe effective rate of DPT group (81%) was better than that of Group B (51%), showing significant difference between two groups (P<0.05).The laboratory indices in Group A were much more evidently ameliorated than Group B .The side reactions of both groups revealed no obvious difference. ConclusionDouble pulse therapy with MP plus CTX is a safe and effective method for the treatment of ALN.
