【摘要】 目的 :探讨尼尔雌醇对老年高血压妇女肾脏的保护作用。方法 :已接受降压治疗的 4 1例老年高血压妇女随机分为尼尔雌醇组 (A组 )和安慰剂组 (B组 ) ,均继续服用降压药 ,疗程为 6个月。观察治疗前后尿转铁蛋白(TRF) ,尿视黄醇结合蛋白 (RBP) ,P选择素 (P Selectin) ,内皮素 (ET 1)和血管性血友病因子 (vWF)的变化。 2 4例健康体检者作为对照组。结果 :①治疗前高血压组TRF ,RBP ,P Selectin ,ET 1和vWF均显著高于对照组 (P <0 .0 1) ;②治疗后A组和B组TRF和RBP均有明显降低 ,前者的降低程度 >后者 (P <0 .0 5 ) ;③治疗后A组血P Selectin ,vWF和ET 1明显降低。结论 :①已接受降压治疗的老年高血压妇女仍存在肾脏损害 ;②尼尔雌醇治疗能明显减少TRF和RBP ;③尼尔雌醇可能通过保护内皮细胞功能以及抗血小板活化发挥肾脏保护作用更多还原

【Abstract】 Objective To investigate the renal protective effect of nilestriol in aged hypertensive women. Methods Forty one aged hypertensive women treated with antihypertensive drugs were randomly divided into two groups. The women in Group A (n=21) received nilestriol, whereas those in Group B (n=20) received a placebo for six months. Antihypertensive drugs were unchanged. Twenty four healthy subjects were set as the normal control group. Results ① Before the treatment, the levels of urinary transferrin (TRF), retinol binding protein (RBP), P selectin, vWF, and ET 1 were significantly higher in the aged hypertensive women than those in the control group. ② The levels of TRF and RBP significantly decreased in both Group A and Group B after the 6 month treatment, but these levels in Group A were significantly lower than those in Group B (P<0.05). ③ The levels of P selectin, vWF, and ET 1 in Group A were significantly lower than those in Group B (P<0.05～0.01) after the treatment. Conclusion ① In aged hypertensive women treated with antihypertensive drugs, the renal function is still damaged; ② Nilestriol can significantly decrease the levels of TRF and RBP; ③ The renal protective effect of nilestriol may be related to the protection of endothelin function and antithrombosis activity.更多还原
