【摘要】 目的　探讨主动脉弓段食管癌外科治疗的有关问题。方法　对 1996年 5月～ 2 0 0 2年 5月 78例主动脉弓段食管癌病人的外科治疗进行了回顾性分析。结果　根治性切除 5 3例 ,姑息性切除 12例 ,切除率 83 3% ,单纯探查 13例占 16 7% ,围手术期死亡 2例。结论　主动脉弓段食管癌周围结构复杂 ,切除率低 ,术中风险性大 ,术后心血管并发症多 ,应加强术前评估 ,半量放疗能提高切除率 ,术中逆行置入金属食管带膜支架是解决单纯探查有效的治疗手段更多还原

【Abstract】 Objective To explore the pertinent problems of surgical treatment for the patients suffered from esophageal carcinoma located at the aorticarch section.Methods From May 1996 to May 2002,78 cases of surgical treatment of esophageal carcinoma located at the aorticarch section were analyzed retrospectively.Results 53 cases underwent radical resection,12 cases received palliative resection (resection rate was 83.3%),13 cases were explored,2 cases died after operation.Conclusions The structures around the esophageal carcinomaner aorticarch part are much more complex.The resection rate is obviously lower and the cardiovascular complications are higher than in other parts and more risky during the operation.The evaluation before the operation should be stressed,the attention should be paid to the local anatomy and careful operation.Semi-quantity radiotherapy before operation can enhance the rate of resection.The effective method for explored cases was stent placement by adverse way in operation.更多还原
