【摘要】 目的 :探讨老年人胰岛素抗体 (INS Ab)的临床特征。方法 :将 84例老年人分为三组 :2 6例糖尿病(DM)患者 ;37例心血管疾病患者 ;2 1例其他疾病患者。采用电化学发光法检测INS Ab ,用放射免疫法检测胰岛素 (INS)。结果 :DM组INS Ab的阳性率明显高于其他疾病组 (x2 =5 .4 99,P <0 0 5 ) ;6 0～ 6 9岁、70～ 79岁和 80～ 89岁 3个年龄组空腹血糖 (FBG)和INS Ab无显著性差异 (P >0 0 5 ) ,但空腹胰岛素 (FBINS)及舒张压差异显著 (P <0 0 5～ 0 0 0 1和P <0 0 5～ 0 0 1) ;INS Ab与FBINS呈高度正相关 (r =0 32 6 ,P <0 0 0 1)。结论 :在非DM患者中 ,可能有与INS Ab相关的免疫失调 ;在老年人 6 0岁以后的几年中 ,不但处于随增龄而增加的高血糖状态 ,而且还可能有高INS Ab和高血压的危险更多还原

【Abstract】 Objective: To investigate the clinical features of insulin antibody (INS Ab) in elderly patients. Methods: INS Ab concentration was measured with electric chemical lightassay and insulin (INS) was measured with radioimmunoassay. The subjects studied were divided into 3 groups, eg. diabetes group (DI) including 26 elder patients with diabetes, cardiovascular group (CA) consisted of 37 elder patients with cardiovascular diseases, and other disease group (OT) consisted of 21 elder patients with other diseases. Results: The positive rates of INS Ab in DI group were higher than those of OT group (χ 2=5 499, P<0 05). There was no significant difference of fasting blood glucose (FBG), but there were significant differences of fasting blood insulin (FBINS) and diastolic pressure among three age groups (P<0 05～0 001, and P<0 05～0 01). Serum levels of INS Ab were correlated positively with serum levels of FBINS (r=0 326, P<0 001). Conclusion: There might occur immunological disorder relating with INS Ab in patients without diabetes. For the elderly patients beyond 60, there might be not only the possibility that the blood glucose level increases with the age, but also the risk of high INS Ab and hypertension.更多还原
