【摘要】 目的评价改良W型切口在特殊情况下白内障手术的临床效果。方法本组患者巩膜瓣切口均做在12点钟,距角膜缘后4～5mm处反眉弓板层切开长5mm,在切口两端向角膜缘延长2条巩膜切口成W型,分离巩膜瓣至角膜缘内1～1.5mm完成巩膜瓣制作。结果术后1d视力增加2行以上者179眼,增加5行以上者103眼,术后1月提高5行以上者125眼,角膜手术性散光为:术后3d(1.50±1.23)D,术后1月(0.99±0.87)D,术后3月(1.05±0.94)D,小梁切除术后,眼压平均为(2.1±0.9)kPa,滤过枕均为扁平弥散型。结论改良W型切口适用于特殊情况下白内障手术,尤其适用于青光眼白内障人工晶状体植入三联手术,是一种具有使用价值的手术方式,值得眼科医生推广使用。更多还原

【Abstract】 Aim To evaluate W-shaped incision used in special cataract surgeries. Methods On 138 cataract cases (187 eyes), scleral flap was made at the 12 o’cl ock position; the anti-superciliary arch 4-5mm posterior to corneal limbus was c ut at a length of 5mm and the incision was extended both ways to corneal limbus to produce 2 scleral cutting in the shape of W; scleral flap was separated to 1-1.5 interior to corneal limbus. Results At postoperative 1d, visual acuity rose by 2 lines or more in 179 eyes and by 5 lines or more in 103 eyes; at postopera tive 1 month, visual acuity rose by 5 lines in 125 eyes. Operational astigmatism was (1.50±1.20)D at postoperative 3 days, (0.99±0.87)D at postoperative 1 mon th and (1.50±1.23)D at postoperative 3 months. After trabeculectomy, intraocul ar pressure averaged (2.1±0.9) kPa and filtering pillow was flat and diffused. Conclusion Modified W-shaped incision can be applied very well to cataract surg ery, especially to combined antiglaucoma, cataract and intraocular lens implanta tion surgery.更多还原
