血小板活化因子对梗阻性黄疸机体细胞免疫功能的影响

【摘要】 目的 探讨血小板活化因子 (PAF)对良性梗阻性黄疸机体细胞免疫功能的影响及其与内毒素、肝损害的关系。方法 健康成年白兔 2 5只 ,随机分为假手术组 (A组 )、胆总管结扎组 (B组 )、胆总管结扎加 BN5 2 0 2 1应用组 (C组 )。术前及术后采集标本检测。结果  1 B组血清和肝组织 PAF、门静脉和体静脉内毒素明显高于 A组和 C组 ,C组门静脉和体静脉内毒素仍高于 A组。2 B组 CD3、CD4细胞数及 CD4 / CD8比值均低于 A组和 C组。B组与 C组中性粒细胞吞噬率、杀菌率无统计学差异。3B组血清 PAF与体静脉内毒素、肝组织 PAF呈正相关 ,肝组织 PAF与门静脉内毒素呈正相关。4术后 C组肝功能和肝组织病理变化有不同程度的改善。结论 梗阻性黄疸时细胞免疫功能抑制与 PAF增高有关。内毒素是 PAF产生增加的主要原因

【Abstract】 Objective To explore the effect of platelet activating factor(PAF) on cellular immunity in benign obstructive jaundice as well as the relation ship between PAF,endotoxin(ET) and hepatic injury.Methods Twenty five rabbits were randonmized into 3 groups:sham operation(A) group,bile duct ligation(B) group,bile duct ligation and PAF antagonist BN 52021 applied(C) group.All samples were collected to study perioperatively.Results (1)Serum and hepatic PAF、portal and systemic ET levels in group B were significantly higher than that in groups A and C.Portal and systemic ET in group C was higher than that in group A.(2) CD3、CD4 cells and the ratio of CD4/CD8 in group B were lower than that in group A and C.Neutrophil phagocytotic rate(PHR)、kill bacteria rate(KBR) in groups B and C were no difference.(3) In group B,serum PAF was positively related to systemic ET and hepatic PAF.Hepatic PAF was positively related to portal ET.(4) The hepatic function and pathologic changes in group C were improved.Conclusion PAF plays an important role in the suppression of cellular immunity in obstructive jaundice.ET is the main derivant that stimulates PAF to be produced.
