【摘要】 目的　探讨肝癌合并门静脉高压症施行联合手术的疗效、手术方式及适应证。方法　对 2 6例联合手术治疗肝癌合并门静脉高压症病人进行回顾性临床分析。结果　 2 6例均恢复顺利 ,白细胞均恢复正常、血小板恢复正常 18例 ,增高 (>5 0 0× 10 9/ L ) 8例。 1、3、5年的生存率为 89.5 %、63 .1%、4 7.4 %。术后并发症发生率 4 2 %。门静脉血栓 3例 ,顽固性腹水 3例 ,轻度肝昏迷 1例 ,胸腔积液 4例。结论　联合手术的术后并发症增加 ,但围手术期死亡率并不增加 ,远期疗效满意。合理的选择手术方式 ,对肝癌合并门静脉高压症施行联合手术是安全的 ,可行的 ,有效的更多还原

【Abstract】 Objective To study the combined operation methcd and indication for hepatocellular carcinoma with portal hypertension.Methods Petrospective analysis was carried out for 26 patients who underwent combined operation on hepatocellular carcinoma and portal hypertension.Results 26 patients all recovered smoothy,white blood cell became normal;blood platelet were normal on 18 patients and increased in 8 patients(>500×10 9/L).Surviral rate of 1?2?3 year were 89.8%,63.1%,47.4% respectively.Postoperative complications were 42%. There were 3 patients occurred portal vein thrombosis,3 patients with intractable ascites, 1 case of mild hepaticoma, 4 patients with pleural effusion.Conclusion If the indications selected correctly,the combined operation on hepatocellular carcinoma with portal hypertension is a safe?feasible and effective method.In spite of its high postoperative complication rate,the perioperative death rate was not increased.更多还原
