【摘要】 目的 :探讨经肾动脉导管超选择至肾段及肾亚段对肾肿瘤及肾动静脉畸形进行栓塞治疗的临床应用价值。方法 :4例肾肿瘤患者 ,经导管超选择至肾段及肾亚段 ,行化疗栓塞术。 8例肾动静脉畸形均经DSA证实 ,经导管超选择性插管至肾段及肾亚段行栓塞治疗。栓塞材料为弹簧圈、无水酒精、PVA或IBCA。结果 :4例肾肿瘤患者节段性栓塞均获得成功 ,治疗后肿瘤缩小 ,碘油沉积良好 ,CT复查肿瘤平均 6个月无复发 ,相邻的肾段无梗死征象。 8例肾动静脉畸形患者超选择性栓塞均获得成功。 1例术后 6个月血尿复发 ,再次栓塞后血尿症状消失 ,其余未见复发。结论 :肾段及肾亚段栓塞是治疗无手术适应证的肾脏肿瘤及肾动静脉畸形的有效方法 ,且能够最大限度的保护正常肾脏 ,并发症少更多还原

【Abstract】 Objective:To study the technique of renal segmental or subsegmental transarterial embolization (TAE) in renal tumor and arteriovenous malformation(AVM).Methods:Four patients with renal tumor were treated by segmental or subsegmental TAE.8 cases of renal AVM confirmed by DSA underwent segmental or subsegmental TAE using metal coils,ethanol,PVA or IBCA.Results:4 cases of renal tumor were successfully treated by segment embolization.The tumors decreased in size where the iodized oil was well accumulated.The number and size of the tumor did not increase within 6 months follow up by CT and no infarction of the adjacent renal segment was observed.8 cases of renal AVM were successfully treated by superselective embolization.Hematuria in only one case reocurred 6 months after embolization and disappeared after reembolization.No recurrence in the remaining cases was observed.Conclusion:Segmental or subsegmental TAE is a useful method without remarkable complication for renal AVM and unresectable renal tumor.更多还原
