【摘要】 目的　探讨口服米索前列醇预防产后出血的价值。方法　采用回顾性资料分析方法 ,对我院2 0 0 1年 1月～ 2 0 0 2年 12月收住院的正常单胎头位足月妊娠经阴道试产者 16 0 0例随机分为两组。研究组 80 0例 ,在第二产程胎头娩出后即口服米索前列醇 6 0 0 μg ;对照组 80 0例 ,在胎头娩出后静脉予催产素 2 0U。观察产后2h出血量 ,产后出血发生率 ,新生儿窒息发生率及用药前后血压变化。结果　产后 2h出血量 ,研究组与对照组分别为 16 8 4 5± 6 0 5 6ml和 188 75± 95 32ml,两组比较差异有统计学意义 (P <0 0 1)。产后出血发生率 ,研究组与对照组分别为 1 2 5 %和 3 75 % ,两组比较差异有统计学意义 (P <0 0 1)。新生儿窒息发生率及用药前后血压变化对比差异无统计学意义 (P >0 0 5 )。结论　口服米索前列醇预防产后出血优于催产素 ,不仅能减少产后出血量 ,降低产后出血发生率 ,且给药方法简便、安全 ,便于推广应用。更多还原

【Abstract】 Objective To investigate the effect of misoprostol on prevention of postpartum hemorrhage. Methods A retrospective study was carried out for 1 600 normal parturients,who were divided into study group(n=800)and control group(n=800). For the study group,600 μg of misoprostol was given orally after delivery of fetal head at the end of the second stage. For the control group,20U oxytocin was injected intravenously at the same time point. The amount of bleeding within 2 hours postpartum was measured. The occurrences of postpartum hemorrhage,asphyxia of neonatal and blood pressure change between pre- and post-treatment were compared. Results The mean amount of bleeding of the study group was 168.45±60.56 ml,which was higher than 188.75±95.32 ml of the control group(P<0.01). The occurrence of postpartum hemorrhage in study group(1.25%)was lower than control group(3.75%,P<0.01). There was no statistical difference of asphyxia of neonatal or blood pressure change between the two groups. Conclusions Misoprostol is more effective than oxytocin in controlling postpartum bleeding. Furthermore,Misoprostol is easy to deliver and suitable for widely applying.更多还原
