肾移植患者外周血穿孔素、颗粒酶B表达与急性排斥的关系
Expression of perforin, granzyme B and acute rejection in renal allografts
【摘要】 目的 :探讨肾移植患者外周血淋巴细胞 (peripheralbloodlymphocyte ,PBL)穿孔素、颗粒酶B的表达与急性排斥反应 (acuterejection ,AR)的关系 .方法 :用流式细胞分析仪(flowcytometry ,FCM )动态监测慢性透析 (chronichemodialy sis,HD) (n =2 6 )、AR (n =1 2 )、肾功能延迟恢复 (delayedgrartfunction ,DGF) (n =1 0 )、肾功能正常 (stablerenalallo graftrecipient,SRAR) (n =1 8)组患者的PBL穿孔素、颗粒酶B表达差异和AR的关系 .结果 :①肾移植术后患者PBL穿孔素、颗粒酶B的表达强度依次为AR组、SRAR组和DGF组 .且AR组较其余 3组有显著性差异 (P <0 .0 5 ) ;②其升高时间比临床上出现AR的症状早 3d左右 .随着AR的逆转 ,其表达也逐渐降低至原有基础水平附近 .结论 :PBL穿孔素、颗粒酶B表达是一种有价值的、无创性的AR的标志物 ,不仅有助于AR的早期诊断 ,并可用来评价抗排斥治疗是否有效的手段之一 .
【Abstract】 AIM: To investigate the relationship between the expression of perforin and granzyme B in peripheral blood lymphocytes and acute rejection in renal transplantation. METHODS: 40 recipients of renal allograft and 26 hemodialysis patients were included in our study. The expression of perforin and granzyme B of peripheral blood lymphocytes was studied by flow cytometry (FCM). The recipients were divided into four groups: group 1: chronic hemodialysis ( n =26) as control; group 2: Acute rejection ( n =12); group 3: delayed graft function ( n =10); and group 4: stable function ( n =18). RESULTS: ① The expression of perforin and granzyme B of acute rejection group was significantly higher than that of the other three groups ( P < 0.05 ). ② The expression increased very distinctly in those who had an acute rejection, and it increased about 3 days earlier than the appearance of clinical symptoms. CONCLUSION : As a non invasive method, perforin and granzyme B expression could be used as diagnostic values for acute rejection with high sensitivity (75.00%) and specificity (92.86%).
