经皮穿刺桡动脉冠状动脉造影155例分析

【摘要】 目的:探讨经桡动脉冠状动脉造影的可行性及评价其效果。方法:收集我院自2001年12月1日至2002年11月5日间经桡动脉冠脉造影患者共155例,术前常规行改良Allen试验,阳性者遂穿刺右(或左)桡动脉,依个人习惯使用5FMedtronicBRACHIAL4.0桡动脉造影导管或5F CordisMPA-2造影导管。结果:手术成功率为96.8%,5例失败,失败原因:2例因血管严重痉挛而无法操纵导管,3例右锁骨下动脉严重扭曲致导管无法到达升主动脉。2例术后发生桡动脉闭塞。无出血、夹层、血肿等血管并发症发生。结论:经桡动脉入路冠状动脉造影成功率高,血管并发症少,患者痛苦少,易于接受,具有推广价值。

【Abstract】 Objective: To get the feasibility of percutaneous transradial artery coronary angiography and evaluate its effect. Methods: Collect the patients of our hospital from December 1,2001 to November 5,2002 to be undertaken transradial coronary angiography, 155cases altogether, modified Allen Test must be done before the procedure, we punctured right (or left)radial artery in patients who are positive, we used 5FMedtromcBRACHL\L4.0angiographycatheteror5F’CordisMPA - 2 angiography catheter according to operator.Results: Transradial coronary angiography were successfully performed in 150cases(96.8%) ,five cases were unsucessful, the reason is: two cases were because of the radial artery spasm severely, and the catheter can’ t be manipulated any more, three cases were because of the right subelavian artery tortuosity and failed to access to the ascending aorta. Radial artery occlusion occurred in two cases after the procedure. NO complications such as bleeding,dissection,hematoma were occurred. Conclusion: The success rate of transradial approach is high, vascular complications are extremly rare,patients go through much less pain,and prefer to this approch.It is worthy to generalize.
