可调式中空钛合金人工椎体动物实验研究

【摘要】 目的 :观察可调式中空钛合金人工椎体对犬脊柱重建及植骨融合的作用。 方法 :1 2只实验犬随机分为对照组 (n =3)及实验组 (n =9)。将其制成腰椎椎体切除模型 ,对照组植入髂骨 ,实验组植入填塞有松质骨碎骨块的可调式中空钛合金人工椎体。分别于术后即刻、术后 3个月、6个月摄正、侧位片 ,观察植入后椎节高度的变化及对植骨融合的影响。术后 3个月、6个月将实验犬分批处死 ,切取相应脊柱单元制成不脱钙病理切片 ,光镜下观察骨融合情况。 结果 :椎体置换术后即刻椎节高度较术前无明显变化 ,术后 3个月较术后即刻显著下降 (P <0 .0 1 ) ,但仍不低于术前高度 ,术后 6个月与术后 3个月相比无显著性差异。术后 3个月 ,人工椎体周围有纤维细胞及少量软骨细胞生成 ,植入后 6个月在人工椎体与骨界面上有大量软骨细胞及部分骨细胞生长。 结论 :可调式中空钛合金人工椎体植入后可有效重建椎节高度 ,在犬人工椎体周围有成骨现象 ;该人工椎体可兼顾术后即刻的稳定性及远期骨性融合的需要。

【Abstract】 Objective:To investigate the reconstruction and fusion effect of newly designed adjustable hollow Titanium artificial vertebral body(AHT AVB) in dogs.Methods:Twelve dogs were randomly divided into control group( n =3) and experimental group ( n =9).After lumbar corpectomy,the iliac bone graft were harvested and implanted into the defected lumbar spine in control group and AHT AVB impacted by the cancellous bone were implanted in the experimental group.The migration of the implants, height of reconstruction segment and bone fusion were observed after different periods.Results:The height of spinal segment decreased 3 months after operation with statistical difference,but still longer than that of pre operation.There were fibrous cells and chondrocytes around the AHT AVB 3 months after operation.Mature chondrocytes and osteocytes were found around the AHT AVB 6 months later.Conclusion:AHT AVB can restore the height of spinal segment and allow for osteofication.It can provide both immediate stability and bone fusion for dog corpectomy spine in a long run.
