保留终板的颈椎前路融合术术后融合节段高度变化小

【摘要】 目的 探讨能更好地维持椎体和融合节段高度 ,并有效防止融合节段前方塌陷的颈椎前路植骨融合术式。方法 对 1 0 2例外伤性及退变性颈椎间盘突出症采用保留终板的颈椎前路减压植骨融合术 ,分别行或不行内固定。分别于术前及术后 1周、3、6个月、1、2、3年摄颈椎正侧位X线片以记录融合时间 ,测量融合节段高度。本组平均随访 2年 6个月 (6个月~ 3年 ) ,并与同体相邻节段平均高度作比较。结果 所有病例均达骨性融合 ,平均融合时间为术后 3.8个月 ,均未发现融合节段前方塌陷。经t检验 ,将术后 6个月 ,1、2、3年融合节段平均高度与同体邻近节段平均高度比较 ,两者无明显差异 (P >0 .0 5 )。结论 保留终板的颈椎前路植骨融合术能较好地保持融合节段高度 ,并能有效防止融合节段前方塌陷

【Abstract】 Objective To improve the surgical treatment to cervical disc herniation that could retain the height of intervertebral spaces more efficiently.?Methods One hundred and two cases of traumatic or non traumatic cervical disc herniation were treated with endplate retaining anterior cervical fusion. X ray films were obtained preoperatively and 1 week,3,6 months,1,2,3 years postoperatively. The height of intervertebral spaces was measured and the time of fusion was recorded. All patients were followed up two year and 6 months(6 to 36 months). The heights of fusion spaces were compared with that of the mean value of upper and the lower spaces of the same patient.?Results All intervertebral spaces were fused without anterior collapses of fusion fragments. The mean time of fusion was 3.8 months. There was no significant difference between the height of the fusion fragment and the mean value of the upper and lower fragments(P> 0.05) .?Conclusion Endplate retaining anterior cervical fusion could retain the height of intervertebral spaces efficiently and prevent anterior column of the fusion fragments from collapsing.
