HBeAg、抗-HBe、HBV DNA共存与PreC/C基因变异分析

【摘要】 目的分析HBeAg、抗—HBe、HBV DNA共存的产生原因、临床特点。方法 Abbott法检测HBV血清标志物,HBV DNA定量测定,PreC/C基因序列测定。结果本模式76%系HBV慢性感染,24%急性感染,均有活动性肝损害;HBV DNA定量中位数4.57 copies/ml,HBeAg(P/N)32.31±87.81,抗-HBe(P/N)0.55±0.26;PreC基因83,C基因14、104、158、203、233多位点基因变异,并产生2个终止码变异。结论检测灵敏度的提高、宿主免疫反应激活、病毒基因变异是产生本模式的原因。

【Abstract】 Objective To investigate the cause and clinical feature of common existence of HBeAg, anti-HBe and HBV DNA positive pattern. Methods Serological markers of HBV were detected by microparticle enzyme immunoassay (Abbott). Quantitative analysis of HBV DNA was performed by PCR and PreC/C gene fragments were directly se-quenced. Results Among these patients with HBeAg, anti-HBe and HBV DNA positive pattern, 76% were with chronic hepatitis and 24% with acute hepatitis and liver injury. Median HBV DNA was 4. 57 copies/ml, HBeAg (P/N) 32.31±87.81, Anti-HBe (P/N) 0.55 ± 0.26. Samples from patients were isolated and several points mutation in PreC/ C gene resulting amino acid substitutions were at site of PreC 83 and C gene 14, 104, 158, 203, 233. Furthermore, there were two stop codon mutations. Conclusion The concurrent HBeAg, anti-HBe and HBV DNA in patients may result from improving sensitivity of test methods and activating immunoresponse and viral mutation.
