【摘要】 目的探讨核分裂指数(MI)在胃肠平滑肌肿瘤(GISMT)良恶性鉴别和预后判断中的作用。方法收集159例 GISMT 的临床病理资料。取病理科存档的石蜡包埋组织块,重新切片和染色。对每一肿瘤,随机地无重复地选择50个高倍镜视野,观察其核分裂像。分析 MI 与肿瘤性质和预后的关系。结果平滑肌瘤和平滑肌肉瘤的核分裂指数分别为0.31±0.15/10HPF 和5.77±2.30/10HPF(P<0.001)。在平滑肌肉瘤中,肝转移和复发者的 MI 值明显高于未发生肝转移和复发者(7.85±2.65/10HPF vs 3.44±1.62/10HPF,P<0.001)。结论 MI是区分良恶性 GISMT 的重要指标,MI 高的 GISMT 可能预后较差。更多还原

【Abstract】 Objective To investigate the value of mitotic index (MI) in judging the malignant degree and predicting the prognosis of gastrointestinal smooth muscle tumors(GISMT).Methods The clinical course and pathologic features of 159 patients with GISMT were analyzed.The number of mitoses seen randomly in 50 high-power fields (HPF) was assessed for each tumor.The correlation among MI,malignant degree and prognosis was studied.Results The MI of Leiomyoma was 0.31±0.15 mitoses/10HPF,and the MI of leiomyosarcoma was 5.77±2.30 mitoses/10HPF(P<0.001).In patients with leiomyosarcoma,the MI of those who had a liver metastasis and/or recurrence of the disease was markedly higher than those who had no liver metastasis and/or recurrence of the disease (7.85±2.65/10HPF vs 3.44±1.62/10HPF,P< 0.001).Conclusions Mitotic index is an important criterion to distinguish if the GISMT is benign or malig- nant.The higher the frequency of mitoses,the worse the prognosis may be.更多还原
