休克期切痂对循环状况的改善

【摘要】 目的观察烧伤焦痂对机体血液动力学及全身氧供、氧耗量的影响。方法通过小型猪35%Ⅲ度烧伤后不同时间切痂植皮的对比,观察了两组动物血流动力学、氧供、氧耗、氧摄取、乳酸等循环指标。结果烧伤后4小时即出现严重血容量不足、氧供不足、乳酸堆积,经补液治疗上述指标在24小时内可基本恢复正常,及早切除焦痂对改善血液动力学指标、恢复正常氧供、氧耗、纠正高乳酸血症有重要意义。结论在复苏成功后实施休克期切痂是有益而安全的。

【Abstract】 Objective To determine the effect of escharectomy in burn shock stage on hemody- namics,oxygen delivery(DO2),oxygen consumption(VO2)and concentrations of lactate.Methods 35% TBSA Ⅲ degree burn swine model was used.16 male minipigs were randomly divided into two groups: escharectomy during burn shock stage group and escharectomy group after burn shock stage.Results The levels of cardiac output and hemodynamic parameters were lowered by 20% to 40% of baseline in the beginning of 4 hours after burn injury.There was also an inadequate amount of oxygen delivery and hyperlactaticemia.All these parameters recovered to baseline within 24 hours after injury with success- ful resuscitation.Escharectomy during shock stage plays an important role in stabilizing hemodynamics, restoring DO2 and VO2,correcting hyperlactaticemia.Conclusion It is satistactory and safe to perform eseharectomy during shock period after successful resuscitation.
