【摘要】 目的观察烧伤焦痂对机体血液动力学及全身氧供、氧耗量的影响。方法通过小型猪35％Ⅲ度烧伤后不同时间切痂植皮的对比,观察了两组动物血流动力学、氧供、氧耗、氧摄取、乳酸等循环指标。结果烧伤后4小时即出现严重血容量不足、氧供不足、乳酸堆积,经补液治疗上述指标在24小时内可基本恢复正常,及早切除焦痂对改善血液动力学指标、恢复正常氧供、氧耗、纠正高乳酸血症有重要意义。结论在复苏成功后实施休克期切痂是有益而安全的。更多还原

【Abstract】 Objective To determine the effect of escharectomy in burn shock stage on hemody- namics,oxygen delivery（DO₂）,oxygen consumption（VO₂）and concentrations of lactate.Methods 35% TBSA Ⅲ degree burn swine model was used.16 male minipigs were randomly divided into two groups: escharectomy during burn shock stage group and escharectomy group after burn shock stage.Results The levels of cardiac output and hemodynamic parameters were lowered by 20% to 40% of baseline in the beginning of 4 hours after burn injury.There was also an inadequate amount of oxygen delivery and hyperlactaticemia.All these parameters recovered to baseline within 24 hours after injury with success- ful resuscitation.Escharectomy during shock stage plays an important role in stabilizing hemodynamics, restoring DO₂ and VO₂,correcting hyperlactaticemia.Conclusion It is satistactory and safe to perform eseharectomy during shock period after successful resuscitation.更多还原
