【摘要】 目的　探讨抗中性粒细胞胞浆抗体 (A NCA)及抗心磷脂抗体 (ACA)与 SL E血管损害的关系。方法　 99例活动期 SL E患者 ,以间接免疫荧光技术检测血清中 AN CA,以酶联免疫吸附方法检测血清中 A CA。结果　具有皮肤血管炎表现的 SL E组 A NCA阳性率为 30 .8% (39例中 12例阳性 ) ,无血管炎表现的 SL E患者 ANCA阳性率为 8.3% (6 0例中 5例阳性 ) ,两组间有非常显著性差异 (P<0 .0 1)。 ACA检测结果显示两组间无显著性差异 (P>0 .0 5 )。同时 A NCA阳性组与阴性组比较 ,ACA阳性组与阴性组比较 ,在肾脏损害、关节症状、雷诺现象及低补体、抗 ds DNA抗体或抗核抗体阳性等方面无显著性差异 (P>0 .0 5 )。结论　检测 A NCA对于 SL E合并皮肤血管炎具有一定意义 ,其对于 SL E合并内部器官血管炎的意义尚需进一步研究。更多还原

【Abstract】 Objective To study the relationship between antineutrophil cytoplasm ic autoantibodies (ANCA) and anticardiolipin antibodies(A CA) and vascular injury in patients w ith SL E.Methods We exam ined A NCA /A CA in the sera from 99patients with active SL E by indirect im m unoflourescence tech- nique and enzym e linked im m unosorbent assay(EL ISA) reapectively.Results The positive rate of A NCA in SL E patients w ith cutaneous vasculitic m anifestation was 30 .8% (12 /39) ,and the positive rate in SL E patients without cutaneous vasculitis w as8.3% (5 /6 0 ) ,the difference between the two groups was very significant (P<0 .0 1) . There was no significant difference in the positive rate of ACA betw een these two groups(P>0 .0 5 ) .These two kinds of antibodies were neither significantly correlated with renal involve- m ent,arthritis,Raynaud’s phenom enon,nor with hypocomplem entem ia,antinuclear antibodies and anti- ds DN A antibodies.Conclusion AN CA detection m ay be useful to identify cutaneous vasculitis in SL E, but the value in the diagnosis of vasculitis of internal organs SL E needs to be further studied.更多还原
