实验性脊柱侧凸病理变化观察

【摘要】 目的:利用家兔脊柱侧凸动物模型,观察椎间盘、椎骨及椎旁肌的病理变化。方法:将21只家兔随机分成3组,每组7只。均用钢丝连接肩胛骨和股骨,制成脊柱侧凸模型。固定3、4、5个月后分批处死,取顶椎椎间盘,用Carbazole法及Woesner法分别测定凸凹侧己糖醛酸及羟脯氨酸含量;取顶椎椎骨,行HE染色,观察椎体骺板变化;取椎旁肌用肌球蛋白ATP酶法染色,观察肌纤维病理变化。结果:椎间盘凹侧半纤维环中己糖醛酸含量低于凸侧;羟脯氨酸含量凸凹侧比较差异无显著性意义;顶椎楔形变,凹侧骺板较凸侧变薄;椎旁肌凸侧Ⅰ型肌纤维含量增多,晚期同型肌纤维聚集。结论:脊柱侧凸凹侧椎间盘己糖醛酸含量下降及椎体骺板变薄,凸侧椎旁肌Ⅰ型肌纤维构成比增加,晚期同型肌纤维聚集,与人类特发性脊柱侧凸病理变化类似,因此推测这些变化可能系脊柱侧凸的继发改变

【Abstract】 bjective:To develop a model of experimental scoliosis in rabbit in order to observe the pathological changes of intervertebral discs, vertebrae and paravertebral muscles. Methods: Twenty one rabbits were randomly divided into three groups(n=7,7,7). Scoliosis was produced by connecting scapula to the ipsilateral major trochanter of femurs with stainless steel wire. Three groups of rabbits were killed respectively at 3,4 and 5 months after surgery.The annulus fibrous in apical vertebrae was isolated for analysis and was separated into two parts: concave side and convex side for contrast. Hexuronic acid (HRA) was measured by Carbazole’s method and hydroxyproline (HP) by Woessner’s method. The apical vertebrae was isolated for staining with hematoxylin eosin and observed. Paravertebral muscles were taken at level of apical vertebrae and stained with myosin adenosine triphosphatase (m ATPase) and observed. Results: The HRA on concave side was lower than that of opposite side. There was no significant difference in the HP between two sides. The apical vertebrae became wedged and the epiphyseal plate of concave side became thinned. There was a significant dominance of typeⅠ (slow) muscle fibers on the convex side of curvature and there was a phenomenon of assemblage of the same type of muscle fiber in the third group of rabbits. Conclusion: The lower HRA of the annulus fibrous and the thinner epiphyseal plate of apical vertebrae on concave side and the dominance of typeⅠ muscle fiber on convex side was affected by scoliosis. The same pathological changes in human idiopathic scoliosis may be secondary effects of scoliosis.
