【摘要】 目的为了避免免疫功能下降和继发感染的发生,在小儿外伤性脾破裂时采用选择性保脾术治疗。方法,采用选择性保脾术治疗16例小儿外伤性脾破裂,Ⅰ度伤8例,Ⅱ度伤5例,Ⅲ度伤3例,其中1例合并有左膈肌破裂。结果本组16例保脾术均获成功,术后恢复顺利,痊愈出院,随访结果,疗效满意。结论在外伤性脾破裂行保脾术中必须遵循"抢救生命第一,保留脾脏第二"的原则。手术成功的关键在于适当掌握手术适应证和术式的选择。更多还原

【Abstract】 Objective To avoid reduction of immunologic function and occurrence of secondary infections in treatment of traumatic rupture of spleen in children.Methods In recent 15 years selective spleen-preserving therapy has been adopted by the authors in treatment of 16 children with traumatic rapture of spleen.Among these cases,8 suffered from degree Ⅰ of traumatic rupture,5 from degree Ⅱ and 3 from degree Ⅲ,and one case was complicated with rapture of left diaphragm.Results The spleen-preserving operation was successful in all 16 cas- es of this group,the postoperative recovery was smooth,and all were cured at discharge.The result of follow-up observation showed satisfactory curative effect.Conclusions During spleen-preserving operation for traumatic rup- ture of spleen,the principle of "life-saving first,spleen-preserving second" should be observed.The key to suc- cessful operation lies in mastering appropriately operation indications and selection of mode of operation.更多还原
