吸入NO对慢性阻塞性肺疾病患者血流动力学、血气及氧运输功能的影响

【摘要】 观察了11例慢性阻塞性肺疾病(COPD)患者吸入 40μl/L一氧化氮(NO)前后血流动力学、动脉和混合静脉血气、肺内气体交换、氧运输功能及高铁血红蛋白(MetHb)的变化。结果见吸入 NO 2、8、15 min时,肺动脉平均压(mPAP)分别下降了 21.97%、23.93%和 23.28%,肺血管阻力(PVR)分别下降了33.35%、28.07%和31.09%(均为P<0.01);停止吸入NO 2 min时,mPAP和PVR恢复至吸入前水平。吸入NO对其余指标无明显影响。提示吸入NO选择性降低COPD患者肺动脉压,且具有速效、不影响血气和氧运输功能等特点。

【Abstract】 The effects of inhaled 40 μl/L nitric oxide (NO) on hemodynamics, blood gas exchange, oxygen transportation and methemoglobin (MetHb) level were studied in 11 patients with chronic obstructive pulmonary disease (COPD). The mean pulmonary arterial pressure was reduced by 21.97%, 23.93%, 23. 28% , respectively, and pulmonary vascular resistance was lowered 33. 35% , 28. 07% , 31. 09% , respectively with the continuous inhalation of NO at 2, 8, 15 min, they returned to the base line in 2 min after discontinuing the inhalation of NO. No obvious effects of inhaled NO on systemic arterial pressure, systemic vascular resistance, alveolar-arterial PO2 gradient, arterial oxygen transportation,coefficient of oxygen delivery, and MetHb level were detected. The data suggest that pulmonary vasculature in COPD is relaxed selectively by inhaled NO, with the characterization of immediate action and little effect on gas exchange and oxygen transportation.
