聚合酶链反应、细胞染色体及细胞学联合检测胸水的价值
The Value of the Detection of Pleural Effusion by Combined PCR Chromosome and Cytologic Assays
【摘要】 利用聚合酶链反应(PCR)、细胞染色体及细胞学三项实验联合检测60例胸水,其中临床诊断为结核性胸水35例,癌性胸水22例,其他胸水3例(乳麋性、风湿性、化脓性各1例)。检测结果:PCR 诊断结核性胸水9例,其中真阳性7例,假阳性2例,其余51例阴性。细胞染色体诊断癌性胸水20例(占癌性胸水20/22),细胞学诊断癌性胸水14例(占癌性胸水14/22)。结果表明:三项实验联合检测胸水不仅快速、方便,而且同时得出结核性和癌性胸水的鉴别诊断,提高了胸水病因诊断率。结果还表明,PCR 对结核性胸水的诊断率不太理想,且有假阳性出现,通过联合检测可避免误诊。
【Abstract】 60 Patients with pleural effusion were examined using PCR,chromosome and cytologic assays.Among these 60 cases,35 cases were diagnosed as tuberculosis pleural effusion by clinical diagnosis,22 cases as cancerous pleural effusion,1 cases as chyle,rheumatic and fes- ter pleural effusion,respectively.Among these 60 cases,9 cases were diagnosed using PCR as tuberculosis pleural effusion in which 2 cases appeared false positiae reaction;20 cases were diagnosed using chromosome assay as cancerous pleural effusion(20/22 of patients with can- cerous pleural effusion);14 cases were diagnosed using cytologic assay as cancerous pleural effusion(14/22 of patients with cancerous pleural effusion).These results indicate that this combinated assay not only is fast and convenient but also makes a distinction between cancer- ous pleural effusion and tuberculosis pleural effusion.Therefore,it raises the pathogeny diag- nosis rate of pleural effusion.From these results,we observed that it was not good to diag- nose tuberculosis pleural effusion by using PCR and some false positive reactions appeared. Because some incorrect diagnosises caused by the false positive reactions can be avoided by using the combined assay,it is of value to clinical diagnosis of pleural effusion.
