蝰蛇毒磷脂时间法测定血浆低分子肝素含量及其临床应用
RVVCT Measurement of Plasma LMWH-Fraxiparine and its Clinical Application
【摘要】 用试管法和仪器法研究了蝰蛇毒磷脂时间(Russellvipervenumcoagulationtest,RVVCT)与血浆低分子肝素(lowmolecularweightheparin,LMWH)含量之间的关系。采用的实验条件为:蝰蛇毒100μl;脑磷脂(1:1000稀释)10μl;氯化钙(0.025mol/L)10μl;37℃水浴时间(仪器法为直接温浴时间)1.5min。上述条件下,血浆LMWH含量与凝固时间成良好的线性关系(直线回归γ=0.96~0.99),批内变异系数0.6%~1.9%,批间变异系数1.7%~2.6%。尚建立了对倍稀释血浆LMWH的检测方法,扩大了检测范围,低分子肝素的最高检测限为2.0AXaIU/ml。使用上述方法对接受血液透析的患者和不稳定性心绞痛患者进行了血药浓度的观察。发现一次静脉注射低分子肝素100AXaIU/kg即能满足血透的抗凝要求。皮下给药每日一次150AXaIU/kg,三次可达到稳态血药浓度,长期给药时可固定剂量,基本不需调节剂量给药。
【Abstract】 A calibration was established to monitor the clinical applicaion fo Fraxiparine in two ways:instrument method and test tube method. A good lined correlation(y = 0.96 ~ 0.99) was set up between plasma Fraxiparine and RVVCT(AXaIU/ml,linear scale) and the corresponding clotting time. The CV intra -assay was 0.6% ~ 0.9%, CV value ofinter-assay was 1 .7% ~ 2.6% .The method with which the palients’ primary plasma was diluted with normal mixed plasma(1:1)enlarged the determing range up to the maximum of 2.0A X aIU/ml. We found it is of some value to use such a LMWH for the patients receiving hemodialysis or the treatment of unstable angina pectoris.
