【摘要】 目的：评价选择性血管造影对血管畸形所致消化道出血的定位和定性诊断意义，以及血管内栓塞治疗的结果。材料和方法：１１例不明原因的消化道出血患者通过血管造影诊断为胃肠血管畸形。本组年龄范围在１５～７０岁之间，平均年龄４８岁，均有反复消化道出血病史，出血量较小时，每次３００～５００ｍｌ之间，较大时在１２００～１５００之间，最大量估计达３０００ｍｌ。血管造影顺序一般按肠系膜下动脉－肠系膜上动脉－腹腔动脉方式。血管栓塞剂选用钢圈、明胶海绵、ＰＶＡ微粒三种。结果：在１１例胃肠血管畸形中，由腹腔动脉干及分支合并肠系膜上动脉干供血的４例，由肠系膜上动脉分支供血的７例；发生在胃及十二指肠圈附近的８例，而发生于空肠下段回结肠段的３例。除５例接受手术治疗外，６例行血管内栓塞治疗。结论：选择性血管造影对于胃肠血管畸形可提供直接的影像诊断依据；血管内栓塞术是值得推荐的胃肠血管畸形首选治疗方法。更多还原

【Abstract】 Purpose:To elvaluate the diagnostic significance of selective angiography in localizing site and guality of gastrointestinal bleeding caused by arteriovenous malformation,and the value of transcatheter vascular embolization.Materials and methods:11 patients with unknown causes digestive tract bleeding were diagnosed as gastrointestinal arteriovenous malformation (GIAVM) by the means of selective angiography.the ranged in age from 15 to 70 years old (mean,48 years old).All the patients had histories of repeatedly gastrointestinal hemorrhage,the smaller bleeding amount per time is from 300 to 500ml,the larger amount from 1200 to 1500ml,the top to 3000ml.The angiography was cmployed in the sequence of inferior mesenteric artery (IMA),superior mesentericartery (SMA),and celiac artery (CA).The vascular embolic agents include steel coil,gelfoam,and PVA.Results:Blood supplied vessel of the 11 cases,4 from the celiac artery system complicated with SMA,7 from the branch of SMA.AVM localized 8 on gastro duodeno ringe and the neighbouring tract,3 on the lower jejunal and the upper ileum tract.Except 5 patients cured by surgical operation,6 cases weretreated with transcatheter vascular embolization.Conclussion:Abdominal selective angiography can raise the direct basis of diagnostic imagine,the technique of transcatheter vascular embolization is worth recommending first tio GIAVM.更多还原
