【摘要】 目的　总结 VCD方案 :鬼臼噻吩甙 ( VM- 2 6)联合环磷酰胺 ( CTX)和地塞米松 ( DXM)治疗 期多发性骨髓瘤 ( MM)的疗效和毒副作用。方法　 2 1例可评价的 期 MM患者中 , A 期 16例、 B期 5例。VM- 2 630 mg/ m2静滴、d1～ 3;CTX2 0 0 mg/ m2静注、d1,3,5;DXM10～ 2 0 mg/ d口服、d1～ 7。 2 8天为一疗程。结果　总反应率为 76% ( 16/ 2 1) ,其中部分缓解占 38% ( 8/ 2 1)、进步占 38%( 8/ 2 1)、无效占 2 4 % ( 5/ 2 1)。全组生存期 3～ 31个月 ;中位生存期 ( MST) 13个月 ,其中 A、 B期中位生存期分别为 11,12个月。全组中位缓解期 5个月 ,呈现反应的中位数为 2个疗程。白细胞毒性反应 0°、 °、 °、 °分别占 10 %、54%、31%、5% ;血小板毒性则分别占 67%、17%、13%、3%。全组胃肠道反应轻微 ,未见心、肝、肾毒性。结论　我们认为 ,VCD方案是治疗 期 MM的有效方法。更多还原

【Abstract】 Objective To evaluate the efficacy and toxicites of combination chemotherapy of teniposide,cyclophosphamide,and dexamethasone(VCD)for stage Ⅲ multiple myeloma(MM).Mehtods Twenty-one patients with stage ⅢMM were treated with VCD regimen.Among21 evalu-able patients,16 were in stage Ⅲ A;5 in stage Ⅲ B.Teniposide was given 30mg/m 2by intravenous(i.v.)infusion on days 1-3;cyclophosphamide 200mg/m 2i.v. on day 1、3、5;dexamethasone 10-20mgdaily p. o. on days 1-7.The treatment was recycled every 28 days.Results The overall response rate was 76%(16/21),including partial response 38%(8/21);improvement 38%(8/21)and failure 24%(5/21).The survival time was 3-31 months;medium survival time(MST) 13 months;MST of MM patients in stage Ⅲ A、Ⅲ B were 11、12months,respectively.The medium duration of response was 5 months;the medium cycle for initial response was 2.Among 21 cases,leukopenia and throm-bocytopenia were detected respectively in 10%、54%、31%、5%、and67%、17%、13%、3%、according to WHO grade 0、Ⅰ、Ⅱ、Ⅲ criteria.The gastrointestinal reaction was mild.No cardiovascular,renal and hepatic toxicites were observed.Conclusion VCD regimen is an effective treatment for stage Ⅲ multiple myeloma.更多还原
