肝海绵状血管瘤组织来源的研究

【摘要】 为研究肝海绵状血管瘤(CHL)的组织来源,通过对10例组织块进行Ⅷ因子相关抗原免疫组化研究,对8例切除CHL组织进行电镜超微结构观察。结果发现:10例FⅧRAg阳性表达。电镜发现血窦壁厚25nm左右,血窦腔大小为30~300μm。血窦壁有完整内皮细胞,其下有丰富弹力纤维,中膜层有成纤维细胞、平滑肌细胞,间质中有极丰富和广泛分布的胶原纤维,造成内、中、外膜层界限不清及弹力纤维排列紊乱,胶原纤维可填充血窦腔,内皮细胞程度不等肿胀,甚至半脱落、脱落状;红细胞可大量渗出至间质中。从而证实了CHL为肝动脉末梢的微动脉畸形病变,胶原纤维大量增生是CHL纤维化病理基础。

【Abstract】 In order to study the histogical origin of cavernous hemangioma of the liver (CHL), the expresion of F Ⅷ RAg of 40 CHL tissue blocks and the ultrastueture of 8 CHL specimens were examined: The result showed that 10 GHL tissue blocks were all F Ⅷ RAg positive. The wall of sinus was about 25 μm thick and the lumen of sinus was 30-300μm wide. The wall of sinus was covered by endothelial cells under which there were abundant elastic fibers. There were fibroblast and smooth muscle cells in the middle layer. In the stroma there were abundant and widespread collagen fibers which blurred the boundaries between the inner, middle and outer layer. The elastic fibers spreaded irregularly. The lumen of sinus was filled with collagen fiber. There were also many RBCs filtrating into the stroma. Our results demonstrated that CHL was caused by the malformation of hew atic arterioles and the pathological basis of CHL fibrosis was hyperplasis of clllagen fibers.
