颅脑包虫病X线、CT、MRI影像特点

【摘要】 目的:为加强脑包虫病X线片、CT、MRI影像中生物学特性认识,提高诊断率。方法:对11例脑包虫病囊肿X线、CT、MRI影像特点进行分析。结果:以“壳状钙化影”及骨局灶改变占54.5%,颈动脉造影显示“握球状”占位病变。CT为巨形多囊、单囊,密度稍高于CSF,子囊密度总低于母囊,无增强多无水肿为特征。MRI、T1WI母囊呈长T1低信号,子囊信号更低,不见囊壁影,T2WI母囊呈高信号,子囊与母囊信号不同,故显界线,囊壁为环线状黑影。生长不同阶段或感染、破裂,母囊及子囊信号均可增高,囊壁可显示不清,易误诊,值得注意。结论:MRI可突出表现脑包虫病的生物学特征,在诊断中较CT更为敏感可靠。

【Abstract】 Objective:To improve knowledge of biological characteristic in X ray CT and MRI image of craniocerebral echinococciasis and diagnostic rate. Methods: Characteristics found in X ray CT and MRI of craniocerebral echinococcias were analysed. Results: On the X ray Cranium plain flim, focal lesion and “shell shaped calific shadow”constitute about 54.5%; CAG show“ball holding shape”space occupying lesion. CT characteristic: thickness of big polycyst and cyst is higher than cerebrospinal fluid; thickness of daughter cyst is always lower than that of mother cyst; there is no space occupying lesion of enhancement and edema around. MRI scanning: On T1, WI, mother cyst shows long T1 low signal; Shadow of cyst wall can’t be seen. On T2, WI, mother cyst shows high signal; signal in daughter syst is different from that in mother cyst, so there is a clean border. Cyst wall expresses as linear shadow. On different growing stages of cerebral hydatid cyst or when there is infection, rupture, signals both in mother cyst and daughter cyst can raise, and cyst wall can be unclear, so attention should be paid in diagnosis. Conclusions: In MRI image, because it can show typical hydatid cyst characteristic, it is more reliable than CT in diagnosis.
