慢性阻塞性肺病患者骨代谢改变的观察
Variations or bone metabolism in patients with chronic obetructive pulmonary diseases
【摘要】 通过对慢性阻塞性肺病患者(COPD)骨代谢指标的观察,并与正常人作统计比较,以了解长期慢性缺氧等因素是否会造成COPD患者骨质疏松,从而在临床工作中给予预防性治疗。方法按性别分组测定COPD患者(男16例,女13例)血清Ca、P水平,放免法测定血清骨钙素(BGP)、酶联免疫吸附法测定晨尿脱氧吡啶酚(DpD),同时测定股骨Neck、ward's和Troch三个部位的骨密度(BMD)。对上述结果进行同年龄段成对数据单尾t检验。结果COPD组血Ca、血P水平较正常组无明显差异;观察组血清BGP较正常组升高(男31%,女40%,P<0.05);COPD组的DpD水平比正常组显著升高(男144%,女156%,P<0.01);COPD组的BMD值降低,男性均值为正常的69.8%,女性为72.5%(P<0.01)。结论COPD患者的骨形成与骨吸收均增高,但骨吸收程度大于骨形成,最终导致患者骨量减少,引发骨质疏松。临床工作中对这类患者除治疗原发病、纠正低血氧外,可预防性地补充钙及VitD,避免骨质疏松,提高患者生活质量。
【Abstract】 In this study 29 patients with chronic obstructive pulmonary diseases (COPD including chronic bronchitis and bronchial asthma) were divided into two groups by sex (13 males and 16 females).Sensitive indices,such as serum osteocalcin(BGP),urinary deoxypyridoxine (DPD) and bone mineral density(BMD), were determined in each case, and compared with corresponding control group, respectively. Both the measured results and statistical tests indicated that BMD levels in COPD patients were much lower than those in normal people, and patients’ DpD values were higher than those in the control groups. On the other hand,there is no statistacally significant difference in BGP index. Based on our observations,it is suggested that long-term chronic hypoxia in COPD patients may lead to abnormal changes in bone metabolism and marked increase in bone absorption,causing reduction of BMD and tendency of osteoporosis,which deserves much attention in clinical treatment.
