谷氨酸脱羧酶(GAD65)自身抗体的放射配体检测法

【摘要】 为建立一种诊断和预测胰岛素依赖型糖尿病(IDDM)的免疫学定量检测指标,改良了Pe-tersen的谷氨酸脱羧酶自身抗体(GAD-Ab)放射配体分析法,以试管内转录和翻译的标记65kD谷氨酸脱羧酶作抗原,以蛋白A凝胶作为抗体沉淀剂,用液闪仪测cpm值。对15例初发IDDM和40例对照的测定结果显示:IDDM组cpm值显著高于对照组(P<0.001);以对照组x±2s作为阳性标准,则IDDM组阳性率为67%(10/15),对照组为2.5%(1/40)。故改良的GAD-Ab放射配体分析法对初发IDDM诊断的特异性为97.5%,敏感性为67%。且操作简便,易于推广。

【Abstract】 To establish a quantitative assay of immunological marker(GAD Ab) for type Ⅰ diabetes(IDDM), Petersen’s radioligand assay(RLA) of GAD Ab was modified as follows:In vitro transcribed and translated GAD 65 was used as a tracer, Protein A sepharose was used as a precipitator of antibody, and the liquid scintillation counter was used to measure cpm value. The assay results of GAD Ab in 15 recent onset IDDM and 40 healthy controls showed that the cpm values of IDDM group were higher than that of the control group(P<0 001); In 10 of 15 (67%) IDDM patients and 1 of 40 controls cpm values were over the positive standard(control group cpm mean values +2s), so that the sensitivity and specificity of RLA for GAD Ab in IDDM were 67% and 97.5%; the method was simple and easy to be applied.
