【摘要】 对１９７４～１９８６年甲状腺乳头状腺癌１５２例的手术治疗进行分析。方法１．分型：包膜内型２３例（１５％），腺内型７１例（４７％）和腺外型５８例（３８％）；２．手术方式分为腺叶＋峡部切除和甲状腺癌联合根治术（功能性和根治性）。结果１０年以上无瘤生存率：总生存率为７４．３４％，其中包膜内型１００％，腺内型９１．５５％，腺外型４３．１０％。结论包膜内型仅行腺叶＋峡部切除即可，腺内型和腺外型拟行甲状腺癌联合根治术（功能性或根治性）。更多还原

【Abstract】 PURPOSE 152 cases of papillary carcinoma of thyroid with surgical treatment during 1974～1986 were analyzed. METHODS These cases were divided into two groups:group A,resection of lobe and isthmus;group B,resection of lobe and isthmus with clearance of neck lymph nodes. RESULTS The 10 years no tumour surviral rate of intra integment type (23),intra gland type (30) and extra gland type (9) in the group A were 100%,80% and 22 2% respectively.The 10 years no tumour survival rate of selective clearance of neck lymph nodes (58) and therapeutic clearance of neck lymph nodes (32) in the group B were 96 5% and 61 5% respectively.The lymphatic metastasis of intra gland type and extra gland type were 73% and 92% respectively. CONCLUSIONS That resection of lobe and isthmus only is enough for the intra integment type.Additional clearance of neck lymph nodes should be taken for the intra gland and extra gland type.Under the condition of no influence of entire resection,functional clearance of neck lymph nodes is the first choice.更多还原
