多区切削法与准分子激光治疗高度近视
Excimer laser photorefra-ctive with multi-zone for high myopia
【摘要】 目的:光学性角膜切削术治疗高度或超高度近视方法较多。本文应用三区切削法对高度或超高度近视进行了初步尝试井以了解多区切削法(multi-zone)之疗效。方法:应用EC—5000扫描型准分子激光器对一组超高度近视(等效球镜—9D以上,共46只眼,B组)通过三区法进行了屈光矫治手术。同时与另一组应用普通方法(65只眼,A组)进行了统计学分析和比较。结果:术后两组角膜上皮化速度基本相同;屈光度回退程度及出现夜间视力障碍者亦无明显统计学差异。与对照组相比,角膜上皮下混浊程度相对较轻。术后6月视力在0.5以上者A组90.77%,B组89.14%。P>0.05。结论:本文结果提示多区切削法可作为治疗高度或超高度近视一种选择。但仍存在一些问题,如屈光度回退等。
【Abstract】 PURPOSE. Methods of superficical corneal ablation using the 193 nm excimer laser for correction of myopia and astigmatism are various. To investigate the effects of the multi-zone technique,photo-refractive keratectomy for high and extreme myopia(spherical equivalent than 9 D) were performed. METHODS:A group of extreme high myopias were treated with three-zone technique using EC-5000, a scanning excimer laser system and compared retrospectively to the group with conventional ablation treatment. RESULTS: Six month follow-up data were availabe for 65 eyes with conventional ablation and 46 eyes with three zone treatment. Postoperative healing was similar for the two groups. 90.77% in group A and 89.14% in group B had an uncorrected visual acuity better than 20/40. No significantly more corneal haze and night vision disturbance was observed and no one lost 1 line of best corrected visual acuity in the group with three zone. CONCLUSIONS: The results suggest that the multi-zone corneal scanning ablations should be an alternative means of treating high and extreme myopia though it is still associated with some problems such as considerable refractive regression.
