静脉瓣膜切除破坏后原位一期静脉动脉化治疗严重下肢缺血的远期疗效分析
FOLLOW - UP RESULT OF VENO - VALVULECTOMY OF VENOUS VALVE DESTRUCTION AND PRIMARY IN SITU ARTERIOLIZATION OF THE VEINS FOR TREATMENT OF ISCHEMIC LOWER LIMBS
【摘要】 在对大隐静脉、胫后静脉瓣膜进行解剖学和生物力学研究的基础上,设计静脉瓣切除或破坏后原位一期静脉动脉化两种新术式,治疗动脉广泛闭塞所致严重下肢缺血患者71例(74条肢体),术后随访1~8年(其中随访5年以上32例),静息痛消失率96.5%,间隙性跛行消失率93.7%,截肢率降至2.7%,获得满意效果。本文报告了两种新术式的远期疗效,并就两种术式的评价、指征、术式选择等相关问题进行讨论。
【Abstract】 Objective:To report the follow - up result of 2 new operations and to discuss their indications and assessment. Method: After anatomical and biomechanic study of the valvular distribution and function of the great saphenous and posterior tibial veins, we performed veno - valvulectomy or venous valve destruction and primary in situ veno - arteriolizaion for 74 severely ischemic limbs of 71 patients with extensive arterial occlusion of lower limbs. After operations, 71 patients (74 lower limbs) were followed up for 1 to 8 years. Results: The results were excelent. Rest pain and intermittent clau-dicaton disapeared in 96. 5% and 93. 7% of the limbs respectively. Amputation rate was lowered to 2. 7%. Conclusion: The 2 niw operations, properly chosen, are effective and safe for the treatment of ex-tenxive arterial occlusion in lower limbs.
