创伤性面神经损伤的临床神经电图研究

【摘要】 通过对69例创伤性面神经损伤的神经电图(ENoG)测定各指标的系统研究,发现面神经受损后,其ENoG完全恢复时间常早于临床恢复时间.面神经损伤后,潜伏时(LT)延迟,在正常值14%以下者可望于损伤后半年内神经功能完全恢复;25%~40%则多可于半年至一年内完全恢复;电反应在伤后15日仍中断者,一年内不能恢复,多留后遗症,LT可作为临床量化诊断分级指标.神经受损早期持续时间(DT)缩短,随着神经传导的恢复,DT延长;最大积分面积(S)及反应波幅(A)均呈降低趋势;病变严重者电反应连续性可中断,其电反应开始恢复时间越早,则其预后越好.

【Abstract】 In order to investigate the relationship between the electroneurographic appearances and the prognosis of the patients with traumatic facial nerve injury, electroneurography (ENoG) of 69 traumatic nerve injuries were studied systematically. The results showed that complete recovery of ENoG occurred earlier than clinical recovery. Latency time (LT) of post-injury delayed. The injury could be expected to recover completely after 6 months when the delayed time was less than 14% of normal value. The recovery would postpone to 6-12 months if the delayed time was 25% to 40% of normal. Complete recovery of nerve function would not occur in one year and permanent sequelae of dysfunction would be resulted if the electric reflection could not be detected in 15 days after injury. The index of LT could be used as a standard in clinical quantitative diagnosis. After injury, duration time (DT) shortened in the early stage and lengthened with the recovery of nerve conduction. There was a tendency for square (S) and amplitude (A) to decrease. For the serious lesinos, the continuity of electric reflection could be interrupted. In this case, the earlier the electric reflection appears, the better the prognosis will be.
