经鼻滴入硝酸甘油对气管拔管时心血管反应的影响

【摘要】 目的观察硝酸甘油对全麻手术后气管拔管所致患者心血管反应的影响。方法 40例全麻术后成年患者随机分为硝酸甘油组(1mg/2m1)和对照组。硝酸甘油于拔管前1min经鼻滴入,记录拔管前2min、拔管即刻及之后1、3、5、10min时心率(HR)和收缩压(SAP)变化。结果硝酸甘油组拔管后SAP无明显变化,对照组SAP明显增加(Р<0.01);两组患者拔管的即刻心率均明显增加(P<0.01);结论经鼻滴入硝酸甘油是一种安全、简单且有效的降低拔管后升压反应的方法。

【Abstract】 Objective To evaluate the effect of intranasal nitroglycerine (NTG) on pressor responses to extubation af- ter general anesthesia. Methods Forty adult patients were allocated randomly to one of two equal groups receiving either NTG (1 mg/2ml), or 0.9% sodium chloride (2 ml), intranasally 1 min before extubation. Systolic arterial pressure (SAP) and heart rate (HR) were recorded before extubation and at 0, 1, 3, 5, and 10 min after tracheal extubation respectively. Re- suits SAP did increase significantly in the control group immediately after extubation (P<0.01). This change was absent in patients pretreated with intranasal NTG. HR increased in both groups immediately after extubation (P<0.Ol and P<0. 0l respectively). Conclusion It was suggested that NTG administered intranasally 1 min before extubation might be a safe, simple and effective procedure to attenuate the pressor response to tracheal extubation after general anesthesia.
