急性肺损伤早期抗凝疗法的研究
THE ANTI-COAGULATION THERAPY FOR ACUTE LUNG INJURY INDUCED BY BME
【摘要】 采用骨髓提取液(BME)静脉注射建立家兔急性肺损伤的动物模型。实验组在实验过程中使用肝素进行抗凝治疗,对照组不进行抗凝治疗。两组动物同时观察生命体征、血液气体分析、X线胸片、及肺病理学检查,分析早期抗凝疗法对急性肺损伤以及由此继发的全身脏器损害的影响。实验结果表明:早期抗凝对肺局部病变影响不大(P>0.05),但能明显提高致伤动物的7h存活率(P<0.01)。表明:1)ARDS的死亡原因不完全取决于肺脏局部的病变程度,可能与肺外脏器的继发性损害有关。2)早期使用肝素抗凝可以减轻全身的损害,提高生存率。
【Abstract】 The rabbit model of acute lung injury was induced by bone marrow extractive (BME) intravascular injection in the dosage of 0. 4 ml/kg divided into 3 times. In each time the dosage was 0. 2 ml/kg at the 0 hour,0. 1 ml/kg at 1. 5 and 3. 5 hours. After that the rabbits appeared fast breathing, decreased PaO2, and lung edema in chest film. The experimental group was given Heparin 300u/kg iv. push and followed by 400u/kg/h iv pump to keep the coagulating time between 25 - 30 min. The degree of lungs and other organs injury was analyzed through vital signs, blood gas analysis, X-ray and pathological change of lungs. The results shows there are no distinguish difference of the results of the lung changes and the blood gas analysis between the two groups (P>0. 05), yet the survival rate of the experiment group is evidently higher than that of the control group (P<0. 01 ). It states; 1.The causes of death in ARDS patients are not completely decided by the pathological changes of the lung, but may be concemed with the secondary injuries of nonpulmonary organs. 2. to anti coagulate with Heparin in an early period may reduce the generalized injury and increase the survival rate.
