全髋置换后股骨部分骨溶解现象分析
Endosteal Bone Lysis Analysis in Relation to the Femoral Components of Total Hip Arthroplasties
【摘要】 从1986年~1993年,对37个病人的44个髋关节实施丁全髋置换手术,其中14个病人的15个髋关节进行了临床随访,平均随访时间为7年。又从这个时期年实施的156个人工股骨头置换的病人中,随机抽出16个骨水泥的人工股骨头置换作为对照组。两组病人,虽然使用相同的第一代骨水泥技术,相同金属材料的假体。但全髋置换组的骨溶解带数为90个,而人工头组的骨溶解带数为6个(p<0.01)。结论是:造成骨溶解带数明显不同的原因,是由于全髋置换组中聚乙烯的杯所产生的聚乙烯颗粒造成的。
【Abstract】 Thirty-seven consecutive patients had a primary total hip arthroplasty between July 1989 and June 1993. Fourteen of the thirty-seven patients, 16 hips were followed prospectively for a mean of seven years. In order to study the reason of the bone resorption, sixteen of the one hundred and fifty-six patients were randomized from the cemented hemiarthroplasties as a control group. Although the both groups, THA and HemiTHA(artificial femoral head)used the seme cement technique of the first generation and the same metal alloy,but the total number of bone resorption zones was big different. (P<0. 01) Conclusion : the distal migration of polyethylene wear debdris appears to be the major cause of bone resorption, loosening and failure of total hip arthroplasties.
