白血病患者自体骨髓移植后性腺内分泌功能的变化

【摘要】 为了解MAC预处理方案的相关毒性──对性腺内分泌功能的影响,观察了19例白血病患者用MAC方案预处理行自体骨髓移植前后的垂体-性腺内分泌功能。所有患者移植前血清卵泡刺激素(FSH)、黄体生成素(LH)、催乳素(PRL)、睾酮(T)和雌二醇(E2)水平与正常对照组无明显差异(P>0.05)。女性在移植后1~12个月血清FSH和LH较移植前明显升高(P<0.01),血清E2水平较移植前明显降低(P<0.01),移植后1年内均有不同程度的闭经;男性在移植后1~12个月血清FSH较移植前明显增高(P<0.01),血清LH和T虽在正常值范围,但与移植前比较仍有统计学差异(P<0.05)。所有患者移植后1个月后血清PRL水平与移植前均无显著差异(P>0.05)。63.6%男性血清FSH水平和66.7%女性血清FSH、LH及E2水平在移植后1年内逐渐恢复正常。研究结果表明,MAC方案预处理自体骨髓移植后,患者性腺受到了一定程度损害,但多数患者可在近期内恢复,不影响长期存活者的生活质量。

【Abstract】 To investigate the toxicities associated with melphalan,Ara-C and cyclophosphamide(MAC)conditioning regimen,pituitary-gonadal endocrine function was studied in 19 patients with acute leukemia before and after autologous bone marrow transplantation(ABMT)with MAC conditioning.Before ABMT,the serum follicle stimulating hormone(FSH),luteinizing hormone(LH),prolactin(pRL),testosterone(T)and estradiol(E2)levels showed no difference from controls(P>0.05).At one to twelve months post-transplant,the serum FSH and LH levels were higher(p<0.01)and the E2 levels were lower than those pre-transplant(P<0.01)in all female patients,while the serum FSH levels were higher than those pre-transplant(P<0.01),and the LH and T concentrations were in normal range in the male patients.All women were amenorrhea within one year after ABMT.The PRL levels in all patients at one year post-transplant showed no difference from those at pre─transplant(P>0.05).The FSH,LH and E2levels in 66.7% of women and the FSH levels in 63.2%of men recovered at one year after ABMT.The results showed that,after ABMT with MAC preparative regimen,the gonadal of patient was impaired,but recovery could occur in a short period
