PCR检测急性早幼粒细胞白血病骨髓移植前后RAR~α/MYL融合mRNA的动态变化
Detection of the RARα/MYL fusion transcripts in AML-M3 patients before and after allogenic BMT
【摘要】 应用四条引物(R5、R6、D3、D4)通过逆转录聚合酶链反应(RT-PCR)对9例急性早幼粒细胞白血病(AML-M3)患者异基因骨髓移植(BMT)前后进行了 RARα/MYL 融合 mRNA 动态监测。建立的方法敏感度为0.1ng,总 RNA 相当于在104~105个正常细胞中检出一个 AML-M3细胞,扩增产物的特异性经 kpnI 酶切法确认。所有9例病例中,3例于 BMT 后由阳性转阴。4例 BMT 后持续阴性表达,此7例患者均获长期无病生存。1例 BMT 后 RARα/MYL 融合 mRNA 持续阳性表达,最终出现白血病复发,1例失访。结果说明 RARα/MYL 融合 mRNA 是 AML-M3的特异性标记,其动态检测有助于 AML-M3患者 BMT 后残留白血病的检测,对 BMT 后白血病复发有预示价值。
【Abstract】 The RARα/MYL fusion transcripts were determined in bone marrow or peripheral blood samples from 9 cases of acute promyeleocytic leukemia before and after BMT by using re- verse transcripts polymerase chain reaction (RT-PCR technique.Four primers (R5,R6,D3,D4) were employed in this study.The test was able to detect the amplified RARα/MYL fusion tran- scripts from as little as 0.1 ng total bone marrow RNA or amount equivalent to one AML-M3 cell per 104~105 normal cells.The specificity of amplified products could he further identified by Kpn I cleavage test.Seven cases showing persistent RT-PCR negative results all had long-term dis- ease-free survival,while one patient with persistent RT-PCR positivity after BMT finally had ex- tramedular relapse.We concluded that RT-PCR for RARα/MYL fusion mRNA was a sensitive and specific tool for monitoring minimal residual disease and for prediction of leukemia relapse in AML-M3.
