尿液假尿嘧啶核苷对原发性肝癌诊断价值的探讨
CLINICAL EVALUATION OF URINARY PSEUDOURIDINE ASSAY IN THE DIAGNOSIS OF PRIMARY HEPATOCELLULAR CARCINOMA
【摘要】 应用高效液相色谱法测定了42例原发性肝癌(PHC)、65例其它各种肝病、11例消化道恶性肿瘤和40例正常对照者的尿液假尿嘧啶核苷(假尿苷,Ψ),PHC组的尿Ψ浓度(7.0±4.2nmol/μmol肌酐)明显高于正常对照组(3.2±0.7nmol/μmol肌酐)、肝炎组(3.3±0.7nmol/μmol肌酐)、肝硬化组(3.0±1.1nmol/μmol肌酐)及肝良性占位病变组(3.0±0.8nmol/μmol肌酐,P<0.01);而与转移性肝癌组(5.8±3.6nmol/μmol肌酐)和消化道恶性肿瘤组(5.0±2.4nmol/μmol肌酐)无显著性差异(P>0.05)。在42例PHC中27例(64.3%)超过正常对照者尿Ψ浓度X±2S(4.7nmol/μmol肌酐),其中5例小肝癌有3例(60%)尿Ψ阳性。尿Ψ对PHC的诊断敏感性和特异性分别为64.3%和85.5%,对AFP阴性PHC的诊断阳性率为61.5%。尿Ψ与血清AFP之间无相关性(P>0.5),尿Ψ与血清AFP联合检测能进一步提高PHC诊断阳性格出率至88.1%。经手术切除或肝动脉插管化疗等治疗有效的7例PHC患者尿Ψ值均降低。结果显示尿Ψ作为一?
【Abstract】 By means of high-performance liquid chromatography, the urinary concentration of pseudouridine(Ψ) was determined in 42 patients of primary hepatocellular carcinoma (PHC), 65 various hepatic disease, 11 gastrointestinal cancer and 40 healthy controls. The mean urinary Ψ concentration was significantly higher in the patients with PHC than those with hepatitis, liver cirrhosis, hepatic benign tumour and healthy controls,(P<0.01), but no statistical difference was found between the patients with PHC and those with metastatic liver cancer, gastrointestinal cancer(P>0.05). UrinaryΨ level was elevated above the mean value plus 2SD of the healthy controls (4.7nmol/μmol creatinine) in 27 (64.3%) of 42 PHC patients, including 3 of 5(60%) small PHC patients. The results revealed that diagnostic sensitivity and specificity of urinary Ψ for PHC were 64.3% and 85.5% respectively. No correlation could be found between urinary Ψ concentration and serum AFP level (P>0.5). The diagnostic positivity was 61.6% in AFP(一) PHC patients. The combination assay of urinary Ψ and serun AFP could further raise the detection rate of PHC up to 88.1%. The results suggest that urinary Ψ can be regarded as an useful biochemical marker in the diagnosis and monitoring PHC, particularly in combination with serum AFP to increase the detection rate in PHC.
