【摘要】 作者通过对１６０例慢性下腰痛（ＣＬＢＰ）患者的立位Ｘ线平片进行测量，结果发现ＣＬＢＰ患者的腰椎曲度减小，与骶骨倾斜角呈明显正相关（ｒ＝０．６６８，α＝０．０１），两者在小于４０岁年龄阶段内随增龄而呈减小趋势，腰椎曲度顶点下移至Ｌ３椎体以下者占５５％．同时发现前凸指数与腰骶角呈明显负相关（ｒ＝－０．４８３，α＝０．０１）．另外比较了同年龄阶段单纯ＬＢＰ与腰椎间盘突出症患者的各测量值，结果发现后者腰椎曲度、骶骨倾斜角和前凸指数明显减小（Ｐ值＜０．０５和Ｐ值＜０．０１），而腰骶角加大（Ｐ值＜０．０１）．本测量揭示了腰椎在维持自身稳定时骨性结构的内在力学关系，说明腰椎间盘突出症患者存在更大的腰椎不稳定，支持了在腰椎退行性变过程中，椎间盘退化是原发的，而小关节退化是继发的观点．更多还原

【Abstract】 Thisstudywasdesignedtoinvestigatethedegenerativepathologyofthelumbarandthelumbo-sacralspinebymeasuringtheX-rayfilmsonthelateralprojection,onstandingpositionin160casesofchroniclowbackpain(CLBP).TheresultsrevealedthatthelumbarcurvaturewasdecreasinginCLBPinpositivecorrelationwiththesacrumslantangle(r=0.668,alpha=0.01).Belowtheageof40,thetwoparametersweredecreasingwithincreasingage.55%ofCLBPcaseshadtheapexoflum-barcurvaturedescendedtobelowthenormallevelofL3.Thelumbarlordoticindexwasinnegativecor-relationwiththelumbo-sacralangle(r=0.483,alpha=0.01).Comparingwiththeparametersbe-tweenthepatientsofsingleCLBPandtheintervertebraldischerniationatthesameagerange,itwasrevealedthatthelumbarcurvature,sacrumslantangle,lumbarlordoticindexwerealdecreased(P<0.01)andthelumbo-sacralanglewasincreased(P<0.01)inthepatientsofintervertebraldischernia-tion.Thisstudyrevealedtheinherentrelationshipoflumbarvertebralbiomechanicsinkeepingitssta-bility,andprovidedevidencesthattherewasmoreinstabilityintheintervertebraldischerniationpa-tientsandthatthedisorderoftheposteriorarticulationswassecondarytothatoftheintervertebraldiscduringthedegenerationoflumbar.更多还原
