【摘要】 采用手握式ｒ探测仪（ＯｎｃｏｐｒｏｂｅⅡ（ＴＭ））及（１３１）Ｉ标记的小鼠抗人胃癌单克隆抗体３Ｈ（１１）进行放射免疫探测的体外模型实验和胃癌病人放射免疫导向手术的临床试验。体外模型实验显示，ＯｎｃｏｐｒｏｂｅⅡ（ＴＭ）探测（１３１）Ｉ的效率为３．３％，探测灵敏度为３×１０５靶细胞，最小分辨距离为１．８ｃｍ。胃癌病人术中探测判断胃壁肿瘤浸润的灵敏度、特异性及准确率分别为９６．４％、９６．７％及９５．９％。探测判断转移淋巴结的灵敏度、特异性及准确率分别为９９．２％、９７．７％及９８．８％。初步结果表明，Ｏｎｃｏｐｒｏｂｅ探测可有效的检出临床癌灶，指导胃癌根治术的施行，提高胃癌根治术的治愈率及改善患者的生存质量。更多还原

【Abstract】 This study was undertaken to define the capacity of radioimmunodetection for minimal deposits of gastric cancer cells in the experimental model as well as clinically in radioimmunoguided surgery(RIGS) by means of a hand held gamma probe.The results from the experimental model indicated that the oncoprobe II(TM) with the help of (131)I produced a detecting efficiency of 3.3%. The probe was able to identify fewer than 3×105 human gastric cancer cells colony in vitro labeled with MAb 3H(11).The minimal distance of discrination was 1.8cm.On clinical trial it was found that the sensitivity accuracy of detecting tumor infiltration in gastric wall was 94. 6%, the specificity was 96. 7%,and the accuracy rate was 95.9%.In the detection of metastatic lymph nodes,RIGS yielded a sensitivity of 99.2%,and a specificity of 97. 7%,and the accuracy rate was 98. 8%.It appears that this oncoprobe is very promising for the identification of obscured deposits of gastric cancer and makes radical resection of all tumor involved tissues a possibility.更多还原
