汉防己甲素和克矽平联合治疗矽肺疗效研究

【摘要】 本文采用口服小剂量汉甲素及经纤维支气管镜向肺组织滴入克矽平联合治疗矽肺,并设立对照组进行临床研究。经2年治疗和观察,症状,体征显著改善,呼吸道感染率明显下降,x线胸片显示进展率明显减少、部分病例好转;好转病例全部出现在石英尘所致的矽肺组,好转率达35.71%,进展率为0(对照组66.67%),提示联合治疗对石英所致矽肺具有明显疗效。铜矿矽肺治疗组进展率为36.36%(对照组62.50%);煤矿矽肺、铁矿矽肺的治疗作用不明显;上述区别可能与致病粉尘性质、治疗观察时间短有一定关系。

【Abstract】 80 cases of silicosis patients from manufacture and mines were selected . Silicotic lesions of all the cases progressed in various degrees in the past three to four years identified by x-ray examinations . 53 cases were treated with polyvinylpyridine - N - oxide and tetrandrine. 1% PVNO 144 ml was administrated by intratracheal instillation for 1 or 2 times and tetrandrine 100 mg b. i. d. per os for 2-years. 27 cases served as control and placebo was given.After 6 courses of treatment compared with control group in treated group the main symptoms of silicosis such as cough .expectoration ,chest pain and dyspnea improved and infectious rate of respiratory system decreased significantly.According to the appearance of chest x -ray films the progressive rate of treated group(13. 21%) obviously lower than that of control group(33. 33%). In 5 of the 53 cases (9. 43%)the opacities on x-ray films diminished in density and size, however all the 5 cases occured in the group exposed to pure silica and it was 35. 71% of the cases in the group. The stationary rate was 77. 36% in treated group and 66. 67% in control group . No evidence of improvement of lung function was shown. The main side effects were pigmentation(72. 0%)and skin itching (8. 0%).
