大剂量顺铂化疗肾毒性与尿中β2-微球蛋白测定

【摘要】 目的:研究大剂量顺铂(DDP)化疗的肾毒性及监测方法。方法:对13例骨肉瘤术后病人,按每平方米体表面积100mgDDP静脉大剂量化疗。化疗前及化疗后96h分别测定血清尿素(UREA)、血清肌酐(Cr)、血和尿中β_2-微球蛋白(β_2-M)的含量。结果:化疗后有10例病人尿中β_2-M含量升高(P<0.05)。结论:根据尿中β_2-M值来观察接受大剂量顺铂化疗的骨肉瘤病人肾小管的损害程度,结果稳定可靠。化疗前尿中β_2-M超过0.25ug/L以上者要停止大剂量顺铂的化疗。

【Abstract】 Ohjective:Reseach into the renal damage of high-dose Cisplatin(9DP)chemotherapy and method of surveillance,Metbods 13 cases of postoperative osteosarcoma patients treated with Cisplatin(DDP)infusion were studied to determine whether urinary β2-Microglobulin(β2-M)is a reliable indica-tor to predict impaired excretion of Cisplatin. Before and after DDP infusions,the levels of urinary β2-M were measured along with UREA,Cr and blood β2-M.Results:After DDP inhsions,urinary β2-M levels of 10 cases were elevated(P<0.05).Conclusions:Urinary β2-M is a sensitive indicator of reabsorption of renal tubules,and especially when levels of more than 0.25ug/L of urinary β2-M are fund to exist,it seems difficult to clear the drug following Cisplatin treatment.
