盐敏感性高血压对盐负荷呈现“低减型”肾排钠反应者的Ouabain抵抗现象
An ouabain-resistant phenomenon in salt-sensitive hypertensives with decreased renal sodium excretion after saline loading
【摘要】 为探讨盐敏感性高血压者盐负荷后“低减型”肾排钠反应是否与钠泵的Ouabain抵抗有关,我们观察了29例盐敏感性高血压患者在基础状态及盐负荷后的肾排钠反应类型、血浆内源性类洋地黄物质(EDLS)水平、外周淋巴细胞钠泵数量与活性等的改变,以及不同浓度Ouabain对培养淋巴细胞钠泵活性的影响。29例中有9例于盐负荷后呈“低减型”肾排钠反应,后者血浆EDLS水平明显升高,但淋巴细胞86Rb摄入率及Ouabain游离结合位点数目并不减少;经体外培养在一定浓度Ouabain基质中,“低减型”肾排钠反应者的淋巴细胞钠泵86Rb摄入率无相应减少。提示对盐负荷呈“低减型”肾排钠反应的盐敏感者钠泵存在Ouabain抵抗现象。
【Abstract】 Abstract To identify if the decreased type of renal sodium excretion after saline loading in salt-sensitive hypertensives (SS) was related to ouabain-resistance, an acute saline loading test was performed in 29 cases of SS and the renal sodium excretion type, plasma endogenous digoxin-like substances (EDLS),peripheral lymphocyte Na ̄+,K ̄+-ATPase activity and quantity at basal and after saline loading and the effect of difference concentration of ouabain on Na ̄+ , K ̄+-ATPase activity of the cultured lymphocytes were measured.Of the 29 salt-sensitive hypertensives, 9 cases expressed as decreased type of renal sodium excretion after saline loading. In the lat ter group of types. the levels of plasma EDLS signifi cantly increased but the lymphocyte (86) ̄Rb uptake rate did not decrease before and after saline loading and 3 ̄H-ouabain binding sites did not reduce too: the cultured lymphocyte(86) ̄Rb uptake rate was not suppressed by different ouabain concentration in medium.These data suggested that the ouabain-resistant phenomenon existed only in the SS with the decreased renal sodium excretion after saline loading.
