5-羟色胺及其拮抗剂和组织胺在心肌缺血中的作用研究
Effects of 5-hydroxytryptamine, cyproheptadium and histamine on myocardial ischemia
【摘要】 测定无症状心肌缺血(SMI)组(15例)、心绞痛组(11例)和对照组(7例)运动试验前后血浆组织胺和5-羟色胺的含量,并观察了5-羟色胺拮抗剂赛庚啶的效应。结果显示:运动前SMI组血浆组织胺含量明显低于心绞痛组和对照组(P<0.05),运动后三组之间差异无显著性(P>0.05),运动前三组血浆5-羟色胺含量差异无显著性,运动后心绞痛组明显高于SMI组和对照组(P<0.05)。心绞痛组(6例)服用赛庚啶后再次运动试验,在两次运动试验各参数和血浆5-羟色胺含量差异无明显性的状况下,6例心绞痛病人中有3例心绞痛症状消失。提示:组织胺在SMI和心绞痛发生中的作用差异无显性,高血浆5-羟色胺可能参与了心绞痛的发生,而心肌缺血时无症状可能与血浆中5-羟色胺含量未升高有关。P<0.01;心绞痛组与SMI组和对照组比较:△P<0.05;SMI组与心绞痛组和对照组比较:▲P<0.05参考文献
【Abstract】 Abstract The study was designed to evaluate the role of plasma 5-hydroxytryptamine (5-HT) and histamine in silent myocardial ischemia (SMI) and angina pectoris,and the effect of cyproheptadium-an antagonist of 5-HT. In 15 patients (group 1) with SMI, 11 patients (group 2) with effort angina and 7 healthy controls (group 3), the plasma levels of 5-HT and histamine were measured by fluorometry both in resting state and immediately after exercise test. No statistical difference in resting plasma 5-HT levels among the three groups was found. However, after exercise,the plasma levels of 5-HT in group 2 was significantly higher than those in groups 1 and 3 (P<0.05). The resting plasma level of histamine in group 1 was significantly lower than those in groups 1 and 3 (P<0. 05), however, after exercise, no statistical difference in plasma histamine levels among the three groups was found. Six patients of group 2 were treated with cyproheptadium, and then exercise test was repeated, all exercise tests were still positive, it was very interesting to find that no angina pectoris was induced in 3 of the 6 patients. The results suggest that painful myocardial ischemia may be related to the increase of plasma level of 5-HT, and painless myocardial ischemia to the unchanged plasma level of 5-HT , while no significant difference was found in plasma histamine levels between painless and painful myocardial ischemia.
