用人工气管重建气管的实验研究及临床应用
Tracheal reconstruction with silicon prosthesis animal experiment and clinical experience
【摘要】 选用11kg~15kg杂种犬,对气管切除后重建气管进行研究,经过三组动物实验证实,用包绕涤纶布且带涤纶布缝合缘的硅橡胶人工气管进行气管重建效果良好,实验大长期存活一年以上;临床应用于晚期气管肿瘤广泛切除气管后的病人亦长期存活。证明作者自制的硅橡胶人工气管组织相容性好、不漏气、易塑形,是较为理想的气管代用品。施行人工气管重建气管时,采用缝针不穿透受体气管内膜、吻合口两端仅缝合涤纶布以及应用带蒂大网膜包绕全人工气管和吻合口等技术,保证了气管重建术的成功。
【Abstract】 The problem of tubular airwav recon-struction after tracheal resection remainsformidable. We have designed a poly-methylethelene silicon tracheal prosthesis withDacron sewing rings.In the amimal experi-ment with implants in 5 dogs ,we observedthat this“ideal”prosthesis was incorporatedinto the surrouding tissues ,airtight and hadthe ability to be mold into various sizes andconfigurations. All the dogs in the controlgroups died of complications. Successful recon-structions were done with the prosthesis after tracheal resection in 3 patients with trachealcarcinoma.In cnd-to- end anastomosis ,theprocedure included suturation of the trachealsubmucosa and the prosthesis sewing ring,aswell as omental-pedicle wrapping. One of thecomplications,the circumferential granuloma,could be treated with intermittent cauteriza-tion or vaporized with laser.
